Who we are and what this covers
MACA Integrated Health & Wellness operates this public website. Our office is at 7035 Beracasa Way, Suite 103, Boca Raton, FL 33433. This policy describes information handled through the website on which it appears. It does not replace the separate Notice of Privacy Practices or other notices that may govern clinical care and patient records.
Information processed when you visit
Cloudflare hosts and protects the website and may process your IP address, request time, pages requested, browser or device details, and security signals to deliver pages, maintain availability and detect abuse. On pages with Cloudflare Turnstile, Cloudflare also processes technical signals needed to check for automated traffic. The security-check page submits a verification token only; it does not submit an appointment or medical message.
Browser storage and tracking
This site stores an essentials-only privacy notice acknowledgment in your browser so the notice does not repeat on every page. You can clear it through your browser settings or reopen the notice from the footer. Cloudflare security services may use cookies or similar technologies. Cloudflare Web Analytics measures page visits and website performance through a browser beacon; it does not record our booking-link click event. Google Analytics and Google Tag Manager are off. With your explicit permission, Meta Pixel sends a PageView from the public English or Spanish homepage only, allowing Meta to process your IP address, browser details and cookie identifiers. It is excluded from treatment, form, booking, admin and staging pages. You can choose Essential only in the footer privacy notice to withdraw permission; Global Privacy Control prevents activation. A local quality-assurance script counts a limited set of link clicks in browser memory only; it does not send that information to MACA or an analytics vendor. We will update this policy before adding optional tracking.
Information you choose to send
The website does not currently accept newsletter or promotional SMS signups. When the general question form is open, it collects your name, email, optional phone number, question topic and message so MACA can respond. Cloudflare Turnstile checks the submission, and the entry is stored in a MACA-controlled Google Sheet accessible to authorized staff. If you follow a campaign link, approved source, medium, campaign and content labels may accompany your question in the restricted Sheet. These labels do not establish that an appointment was booked. The form does not book care. When email confirmations are enabled, Resend processes your email address and a general acknowledgment to send a receipt. Staff receive a notification linking to the restricted Sheet; the submitted question is not included in these emails. If you call, text, or email MACA, we use the information you provide to respond and assist with your request. Please do not send symptoms, diagnoses, medical records, insurance or payment details, or urgent medical concerns through this form, ordinary email or SMS. Use the approved patient channel for clinical information and call 911 for an emergency.
Booking and other websites
Booking links open OptiMantra, which processes information you enter under its own privacy terms and MACA patient workflows. Text links open your device’s messaging app and messages to MACA’s business number are handled through RingCentral. Google Maps or reviews, Instagram and Facebook are separate services that may process information when you choose to open them. Review their privacy notices before sharing information. Simply viewing this website does not enroll you in marketing messages.
When information is shared
Website technical data is processed by Cloudflare to host and protect the site. Information you voluntarily send by email, phone, or SMS may be handled by the communication provider and by MACA personnel responding to you. We may disclose information when required by law or to protect the site and its users. Optional Meta measurement shares homepage visit signals with Meta only after permission. The site does not send form contents or patient records to Meta.
Retention and security
The privacy notice acknowledgment remains in your browser until you clear it or the site changes its notice version. Hosting and security records are retained according to operational needs and provider settings; correspondence is kept as needed to respond, maintain business records and meet legal obligations. Retention depends on the type of record and applicable requirements. We use HTTPS and security controls, but no online service can guarantee absolute security.
Your questions and choices
You may ask about website information associated with you, request a correction or deletion, or raise a privacy concern by contacting us. We may need to verify your identity and retain some information when required by law or healthcare recordkeeping rules. You can clear browser storage in your browser; this does not remove records held through separate clinical or communication services.
